Driving Directions
Northbound on I-405:-Take Exit 20-B (NE 124th St). Stay left on ramp to go to NE 124yh.-At the light, turn left onto NE 124th St.-Cross the bridge. At the second light, turn right onto NE 128th St.-At the second driveway, turn left into the parking lot at 11415 NE 128th St.
Southbound on I-405:-Take Exit 20 (Totem Lake/NE 124th St) and remain in the right-hand lane.-At the light, turn right onto NE 128th St.-At the second driveway, turn left into the parking lot at 11415 NE 128th St.
Once inside: Evergreen Health and Wellness Offices, LLC, Suite 100 is immediately to your right as you enter the lobby. Enter the waiting room and press the #9 button found in the waiting room to indicate your arrival. I will come and meet you at your appointment time.
